Active Vs. Passive Human-In-The-Loop: Keeping Agents Accountable Requires More Than Just Oversight

Passive human-in-the-loop models are currently the primary enterprise SaaS control mechanism for avoiding AI hallucination risk. In this process, humans are not actively intervening but rather have oversight and the ability to intervene if necessary. However, passive oversight creates an illusion of governance, as it signals control while quietly cultivating automation bias and eroding human engagement. In domains such as EHS and industrial operations, this disengagement can introduce material risk.

Passivity introduces risk, as systems that perform cognitive functions on behalf of humans gradually reduce active engagement. Applied to AI workflows, this manifests when agents assume not only execution but also understanding, leaving humans as passive observers. For instance, root cause analysis is explicitly designed to internalize learning rather than simply produce documentation. When that cognitive function is delegated to an agent, organizations lose the tacit knowledge required to prevent future incidents. Sustained human engagement and the retention of subject expertise will depend on active human-in-the-loop models that keep individuals cognitively embedded in the workflow.

Active human-in-the-loop governance requires users to materially shape AI outputs, rather than simply clear approval gates. In practice, this is embedded through workflow steps that require edits, written rationale, evidence validation, content prioritization or selection between alternative model responses. Vendors offering active human-in-the loop approaches span:

  • Moxo, which routes higher-risk escalations to the appropriate reviewers, captures investigation notes in structured audit trails with full decision context, enabling traceable approvals and compliance-ready governance.
  • Zapier, which enables firms to insert pause points – such as approval requests and data collection steps – directly into AI workflows, with each review action logged for compliance and audit purposes.
  • Cobbai, which emphasizes intervention criteria such as confidence thresholds, content flags, human override rights and reviewer interfaces designed to reduce fatigue and preserve judgement.
  • COMET, which requires users to validate knowledge throughout investigation workflows, with agents supporting data retrieval and orchestration but unable to progress without human validation at each step.
  • Duvo, which routes any AI task that requires input into a central Activity Inbox, where users must review notifications and provide responses before assignments can continue.
  •  IFS, which has agentic AI systems that keep users actively involved in querying, validating outputs via reflection steps and interactively exploring the results to guide and confirm final decisions.

Across these examples, the highest-value AI systems are those that structure and enforce human judgment rather than abstract it away. Active human-in-the-loop design maintains accountability, reduces automation bias and prevents the gradual degradation of expertise over time. Vendor evaluation should therefore focus not on how completely a system removes human participation from workflows, but on how deliberately and effectively it embeds it.
